Assessment Team

The Assessment team is part of the SEND services. They manage the process for checking education, health, and care needs over 20 weeks.

What We Do:

  • We help families and schools by collecting information for new referrals.
  • We prepare everything needed for the SEND inclusion meeting at the Local Authority.
  • After the meeting, we tell families and schools what decisions were made.
  • We work with different health services and professionals to keep the assessment process going.

In Meetings:

  • We meet with families to write a draft Education, Health, and Care (EHC) plan.
  • This plan shows the child's strengths and needs, helping them improve in school.
  • We talk with schools about any funding or support needed for the child.

Team's Role:

  • We also run the SEND inclusion forum. This forum includes members from SEND services, health, schools, and other settings. It helps us make clear decisions about assessments, funding, and where children will go to school.
